Cost of Treatment Services:

  • One-Time Registration Fee: Call for details

  • There is a daily dosing fee of $20.00 for all guest patients and a one-time administrative fee of $25.00

  • AMS of Wisconsin, LLC Onalaska accepts cash, Visa, MasterCard, and Discover, as well as money orders.

    Please note that personal checks are not accepted.

  • Driver License Number or State ID Card Number

  • Dosing Level / Medical Status

  • Phase Level & Dosing Schedule

  • Last 3 urinalysis results

  • Dates of guest dosing

AMS of Wisconsin, LLC Onalaska Welcomes Patients Visiting the Area and in Need of Guest Dosing Services.

Visiting patients will find our professional and patient-centered facilities easy to access, and we strive to provide excellent patient care.

  • In preparation for guest dosing, each patient’s home clinic will need to contact us via phone/fax prior to arrival at AMS of Wisconsin, LLC Onalaska.

  • Each patient’s home clinic MUST fax a signed copy of a guest dosing request form, including the signature of the Medical Director.

  • A minimum of 24 hours-notice must be given from the home clinic. If guest dosing is to start on Saturday, all information should be received by noon Friday.

  • A metal lock-box is required for Sundays, holidays, or to pick up regular take-home medication.

  • Any take home medication must be specified by your home clinic.

  • To schedule an intake, call us today or complete the Get Help Now section, and one of our staff will contact you. Please bring a valid photo ID, insurance card, list of medications you are taking, and intake and treatment fees to your intake appointment.v

  • Mon - Fri: 5 - 11 AM
    Sat: 5 AM - 8 AM
    Sun: 6 AM - 8 AM
    Dosing is not permitted after dosing hours.

  • Typically, after the intake day, just a few minutes. (we do not have multiple dosing windows, just one).

  • You must meet with your counselor to complete a Take Home Request Form to be considered for take-home privileges. Decisions regarding take-home doses are made by the Medical Director in consultation with members of the clinical team. Your counselor can provide you with a Take Home Criteria Checklist.

  • AMS of Onalaska, LLC accepts Cash, credit cards, and Wisconsin Medicaid/HMO.

  • The front desk will schedule you with another available counselor.

  • Yes! In order to transfer to AMS of Onalaska, LLC, you or your current clinic must provide documentation for the following in advance: discharge/transfer paperwork. The intake team must review these documents before you can begin dosing at AMS of Onalaska, LLC, along with being seen by a Clinic Physician, Nurse, and Counselor.

  • If you are going to be out of town and unable to attend the program for medication, you must inform your counselor as soon as possible so your counselor can make an effort to arrange dosing at a program. Each program has different requirements.
